The guidelines depend on your employer, but in most cases, you are eligible to submit the matching gift form within six months to one year of making a gift. Contact your employer's human resources office for a matching gift form and send it to USM.

Some companies require a Federal Tax ID number for the recipient organization when requesting a matching gift. USM's Tax ID is 39-6076442.

Contact your employer's human resources office for instructions. Many companies provide paper or online forms for gift matching. If your employer uses a paper form, please complete your section and mail the form to USM with your donation to the following address:

If your employer uses an online form, USM will receive a notification from your employer that the application for a matching gift was submitted. Please use advancement@usm.org for all emails related to matching gifts.
